What is a Form ID?

A form ID is a number associated with a unique form instance. The format is Year - Julian Date - Submission number of that form instance.

  • Example: 22-245-0001
    • Year: 2022
    • Day: 245
    • First submission of that form for the day 

Do my staff need an email address to use Provision?

No, emails are optional if you would like your users to get email notifications. Otherwise, they will be notified via in-app notifications.

I assigned Tasks to other people, but I can't see them. Where did they go? 

In the Tasks tab, users can see Tasks that are assigned to them individually or a team the user is a part of. If you would like to see tasks assigned in your organization, go to Schedules.

Why am I not receiving my Reviewer Tasks?

There could be one of two reasons you are not receiving your Reviewers tasks:

  • Check to ensure you are a part of the Reviewers team (see How To: Manage Teams)
  • Confirm your notification settings. See How to Manage Notifications.
  • If you are on the Reviewer team and still not receiving Reviewers Tasks, the form rules may not be set up for Review. Please contact us and we will update the workflow rules. 

How to I assign one Form (Task) to multiple people?  

If you would like a group of people to complete one record, assign the Task to a Team.

My employee forgot their password, how to I give them a new one?

Email Accounts:

  • If your employee has an email account, they can select "forgot password" during the login process.

  • If your employee would like to update their password when logged in, see How to Change Your Password.
