➕ How to Create a New Form

Set up a new form’s basic information before you build it


  1. Navigate to the Forms module.
  2. In the top right corner, select the green + Add Form button to open a new form.
  3. On the Information tab, complete the Basic Information section:
    • Name: Enter a name for the form.
    • Sites: Select the sites the form will apply to.
    • Teams: Select which team will have access to start new submissions of the form.
    • Program Code: Select a program code to associate with the form.
    • Risk Category: Select a risk category used for calculating risk associated with the form.
      Note: This is most often left blank.
    • Description: Enter a brief description of the form.
      Note: The description is optional.
  4. Click the green Save button.
  5. Select the Form Builder tab to begin configuring the form.

Your form is created and ready to build, with its basic information saved.
